Request a Teacher Evaluation

Parents, please copy and share the appropriate Teacher Evaluation Form link below with your child’s current math and language arts teachers. If your child only has one teacher, they may complete both sections. Be sure to let the teachers know which grade your student is applying for at AHB.

This form must be submitted directly from the teacher (a current, non-family member). We must receive the completed form before scheduling a classroom day visit.
